Abnormal Nipple Discharge

Nipple discharge is a common breast complaint. A majority of nipple discharge is benign

In favor of benign: bilateral, multiductal, occurs with breast manipulation

Concerning: spontaneous, bloody, unilateral, uniductal, associated with a mass, >40yo



Physiologic:

straw colored, green, brown

galactorrhea (milk)- can be normal up to 6 months after breast feeding

Pathologic:

Bloody- papilloma (52-57%), this is from ductal lining

Ductal ectasia (14-32%)

Malignancy (typically DCIS)

Work- up:

History and Physical

- breast exam and node exam

GAIL Model

Age (>35)

History cancer, DCIS, LCIS,

Age menarche

Age first live birth

Fam history w/ ca (1st
degee)

Prior breast biopsy

Imaging:

30yo or greater = mammo + u/s

<30yo =u /s

***Ductography, MRI

Core needlebiopsy

Tx:

If concerning…

Surgical

Lesion not localized:

complete subareolar duct excision  Lesion
Localized:

lacrimal -> prolene suture-> angiocath ->methylene blue-> isolate and remove duct.

***make sure to remove ENTIRE duct
